  • If you have a Windows 95 format, a floppy disc with the enrollment application will be provided upon request to be filled out and returned by the enrollment cut-off date.

    If internet e-mail is available to you, an enrollment form may be forwarded via an email attachment upon request.

IMPORTANT: There will ba a charge according to level for all make-up tests (based on enrollment fee) payable to the teacher administering the test at the time of the make-up examination. Level 2 or above Theory or Performance make-up requires FULL enrollment fee for EITHER section.
